CHIROPRACTORS AND CHIROPRACTIC ASSISTANTS

(as revised by Regulation on August 6, 2008 pursuant to COMAR 10.43.06)

 

Chiropractors and Chiropractic Assistants:

·        Application fee  

 $200.
·        Examination fee       

 $300.
·         Licensure fee   

    $200.
·         Re-examination fee        $400.
·         Renewal fee           $700.
·         Late renewal fee (in addition to renewal fee)         $500.
·        Reinstatement fee (in addition to renewal and late fees for $200.

each renewal period in which license was expired) 

$300.
·         Duplicate license fee   $  50.
·       

  Duplicate license fee ordered at time of  license renewal 

 $  25.
·         Inactive status renewal fee $350.
·         Reactivation fee (to move license from inactive to active)    

$200.
·          Preceptorship application fee $300.
·          Extern application fee   $  50.
·         Licensure by credential fee (includes background check) $750.
·         Penalty for returned checks   $50.
·        Formal verification of licensure, with seal  $  35.
·        Chiropractic Assistant examination fee  $300.
· Chiropractic Assistant registration fee $100.
·        Chiropractic Assistant renewal fee      $250.
·        Chiropractic Assistant late renewal fee   $200.
·        Chiropractic Assistant re-examination fee     $400.
·         Penalty for failure to maintain correct address with Board $200.
·        Supervising Chiropractor application fee    $300.
·        Continuing Education Course processing review fee (per hourly course unit)    $25.
·         Mailing labels or roster

·        Paper copy of law and regulations    $25

                                

REFUNDS: 

Fees are non-refundable, with the exception of the examination fee.  To receive a refund, the applicant must request a refund, in writing, at least 14 days prior to the scheduled examination.
