The following is only a SUMMARY of public orders of formal disciplinary cases decided by the Board. Full copies of the orders may be requested in writing. This summary should NOT be considered a formal, updated, accurate verification of the license status of chiropractors. Formal verification must be requested in writing from the Board, accompanied by a check for $25.00 per verification:

3.	Baron, F. Steven	S01288	10/14/04	Utilizing unregistered personnel to perform chiropractic assistant services; Failure to maintain adequate patient records: $2,872.00 hearing costs, Formal Reprimand: Upheld on appeal at Circuit Court & MD Court of Special Appeals
4.	Blaker, F. Keen	01074	10/3/01	Violation of Board rules and regulations. Suspension – 1 year Fine - $5,000.00
5	Braxton, Lakasha	S01948	8/19/05	Fraudulent use of license; Unethical Conduct; Filing false report/record; Practice with an unlicensed individual; Unprofessional conduct; Submitting false statements to collect fees. Formal Reprimand; Take/pass Board approved ethics course and jurisprudence examination; pay all hearing costs.

8.	Harris, Wayne	S1506	10/11/01	Prior felony conviction for mail fraud: Ethics course, 100 hours community service, SPEC and MD jurisprudence exams.
9.	Helschein, Steven	S01146	12/15/98	Failure to properly document treatment, Unauthorized personnel: $750 penalty, cessation of practice for 6 months, 2 years probation, mentorship, audit, documentation and risk management courses.
10.	Hughes, Joseph	S01182	11/16/05	Submitting a factually deficient renewal form - failure to disclose pertinent information on 2005 biennial renewal application: 90 day suspension (60 days stayed); 3 years probation; payment of $1,139.00 in hearing costs; take/pass Jurisprudence and Ethics Courses. Affirmed on appeal by Baltimore County Circuit Court on 04/27/2007. Order affirmed by MD Court of Special Appeals. Request for stay of sanctions denied on 06/08/2007.

13.	Kennedy, Patrick	S01511	7/11/00	Inappropriate conduct with female patient: 1 year license suspension, stayed, providing he does not return to practice in Maryland.
14.	Lawrence, Robert	S01095	10/14/04	Summary Suspension of License for incapacity to practice: license on indefinite suspension - October 2004
15.	Miller, Keith	S01427	2/10/00	Unethical, unprofessional conduct, impaired ability: 11/7/01 - Dr. Miller agreed to surrender his license for two (2) years & to withdraw his appeal of the Board’s Order. Should he apply for reinstatement, at end of 2 years, must demonstrate to Board that he is competent to practice chiropractic and that he possesses good moral character.
16.	Mills, Henry	S01132	3/25/00	Ingestion of over-prescribed medication, violation of Board reporting requirements: $5,000 fine, required professional counseling substance abuse monitoring.
17.	Nichols, Clyde	S01285	1/28/99	Improper Billing: Required ethics and record courses and audit.
18.	Regan, Brian	S01350	9/10/95	Unethical practice, False reports, Unauthorized personnel: $5,000 penalty, ethics course, 2 year suspension, all stayed except 6 months, 1 year probation.
19.	Robinson, Daniel	S01364	8/24/99	Unprofessional Conduct: Reprimand, $3,000 penalty, required ethics and records courses.
20.	Rodriguez, Asher A.	S01655	2/22/07	Behaving immorally in practice of chiropractic by committing acts of unprofessional conduct – inappropriate touching of minor female patient: 6 month suspension, 2 years probation, payment of all hearing fees ($3,287.00), take/pass NBCE and Board ethics/jurisprudence exams, mentorship and payment of all associated fees. (Order Affirmed by Baltimore County Circuit Court on 08/04/08).
21.	Rogers, Morgan	S01574	1/28/99	Improper Advertising: Reprimand and requirement to submit an accurate resume.
22.	Romano, Phillip	S01881	10/20/05	Practice with an unlicensed individual. Probation for one (1) year; Pay $1,000.00 fine; Take/pass Board approved ethics and record-keeping course.
23.	Sable, Kenneth	S01765	11/15/07	Violating advertising regulations, including using false/misleading advertisements regarding spinal decompression device, using unapproved practice trade name, unethical conduct, incompetence. ,misrepresenting the effectiveness of treatment, making false reports, paying a fee for referrals. Action: $4,000.00 Monetary Penalty, 6months suspension, 2 years probation, 1 year mentorship with quarterly progress reports, payment of all mentorship fees/costs, jurisprudence exam.
24.	Schaeffer, John	S01757	1/09/06	Unethical Conduct: Failure to reimburse patient for services not delivered. Formal Reprimand, Reimbursement of $709.00 to patient, Payment of all costs associated with Order.
25.	Shefts, Leonard	S01690	12/14/00	Billing irregularities: $500 penalty, record course.
26.	Silverman, Mitchell	S01450	12/22/03	Violation of regulations – failure to maintain proper patient record & entries: 18 months probation, CPT and record-keeping courses.

Tobin, Melissa	S01531	11/14/02	Making a false report; practicing chiropractic with unauthorized personnel; unprofessional conduct; submitting false statements to collect fees for which services not provided: $10,000.00 fine, ethics & record-keeping courses, therapist counseling, random records review.

30.	Young, Timothy	S01948	5/10/04	Probation Lifted - June 13, 2005 Improper Recordkeeping: Two (2)years probation, board imposed mentorship, $5,000.00 penalty, recordkeeping course, jurisprudence course.
31.	Young, Timothy	S01948	11/05/07	Violation of various laws and regulations. Voluntary, permanent surrender of Chiropractic license in lieu of disciplinary hearing. All charges/allegations are considered as proved notwithstanding the absence of an admission by Dr. Young.
